Unlocked is a program led by a team of community health workers (CHWs) committed to providing quality resources to individuals reentering the community after incarceration. We provide support and connection to services that will help individuals be healthy and successful after their transition out of Bexar County Jail.

Eligibility

Meet all of the following criteria:

• Over the age of 18
• Been released from Bexar County Jail in the last three months
• Need health or social service support
