Patient Portal

The Metro Health Patient Portal is a secure online option that gives patients convenient, 24-hour access to personal health information from anywhere with an internet connection. The portal supports new and existing patients by providing records and information on:

  • Immunizations
  • Sexually Transmitted Infections (STIs)
  • Tuberculosis

Access Patient Portal

New Patients

Use the Registration Code and Password provided to you at the clinic. Keep this information safe and confidential.

  1. Click on the blue Sign Up button.
  2. Enter the Registration Code and Password provided by the clinic.
  3. Choose a User Name, Password, Secret Questions and Answers.
  4. Click Submit.
  5. The system will prompt you to change your password.

Returning Patients

  1. Use the User Name and Password provided during registration.
  2. Select the clinic from Practice City drop down.
