Center for Policy & Health Improvement (CPHI)

The Center for Policy & Health Improvement (CPHI) advances systems change that improves health outcomes and equity across Bexar County. CPHI serves as a hub for policy development, equitable community engagement and data-driven solutions that strengthen community health.

CPHI supports local nonprofits, coalitions and public health partners through:

  • Systems change coaching
  • Capacity-building grants
  • Facilitation and training
  • Participatory data practices
  • Story-based advocacy

Our work focuses on strengthening collaboration, aligning strategies, and supporting long-term, sustainable improvements in community health.

CPHI helps coordinate and support implementation of the Community Health Improvement Plan (CHIP) across Bexar County. Using data from the Community Health Needs Assessment (CHNA), our role is to mobilize City of San Antonio Departments and community partners to:

  • Align around shared priorities
  • Coordinate strategies
  • Measure progress and outcomes
  • Improve health through systems and environmental change

Assessments and evaluations guide why and how Metro Health and community partners collaborate to improve health outcomes.

Focus Areas & Assessments

CPHI leads and supports key assessments and initiatives that inform policy and practice, including:

  • Access to Health Care in Bexar County During COVID-19
  • Food Access Assessment (conducted by UTHealth Houston School of Public Health)
  • Outreach and Engagement

In response to COVID-19, City Council allocated funding to support these efforts and established three offices—now unified under CPHI—to create a sustainable community health response.
