Register a Home Birth

The Vital Records office only registers births for the City of San Antonio and Bexar County. To get a birth certificate for a baby born without a medical professional, you and your baby must visit the Vital records office.

NOTE: If the baby was born in the EMS or on hospital property, the birth is not a home birth. The hospital must register the baby.

Required Information

All information and documents must be gathered within five days from the date you notified Vital Records:

  • TxEVER Birth Worksheet
  • Name of the child's current doctor
  • Address of the child's current doctor
  • Medicaid paperwork (if applicable)

Additional Documentation

Provide evidence of pregnancy.
  • Provide prenatal visit documentation.
  • Provide doctor visit summary.
  • Or notarized statement from a licensed, registered, or certified health care provider who is qualified to determine pregnancy as part of the scope of his or her license, registration, or certification.
Provide evidence the child was born alive.
Provide evidence the child was born in Bexar County or San Antonio Texas.
  • Provide utility bill for the month the child was born in the name of the Mother for the address where the birth occurred.
  • Provide mortgage or rental contact/ lease for the month the child was born in the name of the Mother for the address where the birth occurred.
  • If the birth occurred outside of the Mother's primary place of residence, evidence needs to be a notarized affidavit along with a photocopy of ID from a person having knowledge of the Mother's presence in Bexar County or San Antonio on the date of the birth.
Provide evidence the child's birth occurred on the date stated.
  • Provide Admitting paperwork on both Mother and child from hospital.
  • Wellness check documentation for the child.

NOTE: Only one notarized statement is accepted as one of the four evidence sections listed above.

NOTE: Both parents need to be present for the appointment to be listed on the birth certificate.

Proof of Identity

Both parents (if applicable) must present a valid form of identification.

Visitor Security Information

All visitors must pass through a metal detector upon entry. Please allow 15 additional minutes for screening before your scheduled visit.

To help the process go smoothly:

  • Bring small bags only (no large bags or backpacks).
  • Limit your group to 2 - 3 people.
  • Remove any prohibited items before arrival, including metal nail files, pocketknives, firearms, box cutters, scissors, razor blades, pepper spray, stun guns, tasers, or any item designed or modified to function as a weapon.
