Office Holders & Candidates: Personal Financial Statements (PFS)

The State/City Code of Ethics requires Candidates and Officeholders to submit the forms below. Any information submitted on these forms is subject to open records request.

There are two required forms:

Personal Financial Statement (PFS-LOCAL 2026) (PDF)

The Personal Financial Statement form is required by the Texas Local Government Code Chapter 145. This electronic fillable form cannot be printed until it has been completed. If you prefer to handwrite the information, you can print a blank form.

City Personal Financial Statement Addendum

The City PFS Addendum form is required by City Code 2-74.

NOTE: The Office of the City Clerk is not authorized to assist with or advise on PFS forms.

Requirements

PFS Forms must:

Be notarized prior to submission.

The Office of the City Clerk provides notary services for $10 with proper identification.

Be submitted in person or by mail.

City of San Antonio Information Technology Security Policies prohibit transmission of completed forms by email.

Submit the Forms

Mayor, City Councilmembers, City Manager & City Attorney must complete and submit these forms to the City Clerk by April 30, 2026 or request an extension in writing from the City Clerk prior to the deadline.

Failure to Comply

The Texas Local Government Code requires the City Clerk to maintain a list of the municipal officers and candidates for municipal office required to file a financial statement. Not later than the 10th day after each applicable filing deadline, the City Clerk will provide the City Attorney a copy of the list showing whether the officer or candidate timely filed a financial statement.
