Youth Case Management

The Youth Case Management Program provides case management and service navigation to assist youth and young adults 24 years old and younger transitioning out of unsheltered homelessness.

To be eligible for services, youth and young adults must:

Case managers connect the youth and young adults to:

  • education services
  • mental health services
  • local transportation
  • application and move-in fees
  • moving costs (to aid in relocation into stable housing)
  • utility assistance
  • security and utility deposits

The Youth Case Management Program is funded by the Texas Department of Housing and Community Affairs Homeless Housing and Services Program, Youth Set-Aside funding.
