Community Health Worker Hub

According to the Centers for Disease Control and Prevention (CDC):

"A community health worker (CHW) is a frontline public health worker who is a trusted member or has a particularly good understanding of the community served. A CHW serves as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery."

Here in San Antonio, we know them by many names: promotores de salud , peer educators, community organizers for health, patient navigators and many more. They are your neighbors, friends and family who advocate for better health in your community.

Certification & Continuing Education

The goal of the CHW Hub Training Program is to increase training and build community resilience to address health disparities.

It aims to:

  • Fortify the CHW's capacity to provide services and support to priority populations.
  • Increase the social awareness, reach, and impact of CHWs.
  • Empower community health workers to lead to a stronger workforce.

San Antonio Community Health Workers Association (SACHWA)

The SACHWA strives to provide representation and continuing education for certified community health workers and to promote and foster collaboration across disciplines.

Membership and services provided are CHW-focused. Membership is open for certified, non-certified CHWs and CHW students.

The SACHWA offers the following services:

  • Continuing education/professional development
  • Interdisciplinary collaboration opportunities
  • Community engagement/health promotion opportunities

Bexar County CHW Partnership (BCCP)

BCCP has a vision to have a more valued community health workforce and invites local CHWs to join their monthly virtual meetings.
